It should not have ignited when two plant operators opened the gauging hatch on the morning of December 19, 1982. And if it somehow did ignite, it should not have produced a boilover, because heavy fuel oil lacks the volatile components that make boilovers possible. The Tacoa disaster violated both of these theoretical safeguards, killing over 150 people and becoming the deadliest tank fire in recorded history.
